  • Ability to work both independently and collaboratively within a multidisciplinary team.
  • Strong written and verbal communication skills.

Responsibilities

  • Orient clients to the program, including program goals, rules of conduct, and client rights upon admission.
  • Ensure insurance authorizations are accounted for in alignment with dates of treatment and services provided.
  • Provide education and information to clients individually and in group settings on substance use disorders and recovery resources.
  • Coordinate case management needs, matching services, agencies, and community resources with clients to assist with discharge planning.
  • Provide crisis intervention services in response to clients experiencing acute emotional or physical distress.
  • Support clients in identifying strengths, challenges, and needs to inform ongoing treatment planning for substance use and/or mental health concerns.
  • Identify client needs that exceed the current level of care and assist with referrals to appropriate support systems and community resources.
  • Participate in multidisciplinary team meetings to report on client progress and modify established treatment plans as needed.
  • Formulate clinical interventions to promote client stability, engagement, and investment in treatment.
  • Provide counseling for family members or significant others as appropriate.
  • Complete all required documentation accurately and on time, including progress notes, referral and contractual networking records, and incident reports.
  • Conduct initial client assessments for placement and ongoing assessments for continued treatment or referral to a more appropriate level of care.
  • Establish individualized treatment plans with clients, incorporating targeted goals, objectives, and interventions that maximize strengths and available resources.
  • Utilize individual, group, and family counseling skills to help clients explore problems, examine attitudes and feelings, consider alternative solutions, and make meaningful decisions.
  • Chart assessment and treatment planning results, writing reports, progress notes, discharge summaries, and other client-related documentation in a timely manner.
