Requirements

• Knowledge acquired through completion in social work (SW) from an accredited school of social work.
• Unrestricted licensure in social work required.
• At least one (1) year of related experience in a hospital setting, with psychiatric experience preferred.
• Successful completion of CPR and Crisis Prevention Intervention Training within 30 days of employment.

Nice To Haves

• LMSW, MSW, or LPC preferred.
• Will consider CSW or PLPC with stipulations.

Responsibilities

• Complete psychosocial assessments of new patients and develop updates or detailed reassessments of the patient.
• Maintain contact with family and significant others when appropriate, and provide patient and family education, support, and advocacy.
• Provide coordination and act as a liaison with community-based social and mental health agencies.
• Facilitate discharge planning and conduct post-discharge follow up to ensure patients are linked with community resources, reducing re-hospitalization.
• Facilitate therapeutic groups with patients daily.
