About The Position

Our Licensed mental health (MH) professional works as part of a multidisciplinary team to provide the highest quality of care to our patients. The MH professional engages in identification, support and resource management regarding various sources of diabetes distress. The MH professional will provide patient-facing services (1:1 with potential for group) related to the patient's diabetes distress. They will also recommend referrals to local services as their service does not provide long-term or high-acuity care beyond the scope of diabetes distress. They will also act as primary support to the care team for guidance on any urgent MH escalations that may arise during the patient journey. This is a full time, remote, home-office based role eligible for a full benefits package including paid holidays, PTO, and competitive pay in a mission-based work environment. A private, quiet, home office space is required. The schedule for this role is Monday-Friday from 11am-8pm Eastern. This role is open to candidates residing anywhere in the United States who are legally authorized to work in the U.S. on a full-time basis, now and in the future, without employer sponsorship.

Requirements

  • Master’s Degree in Social Work, Counseling or related field from an accredited university required.
  • We are seeking a licensed mental health professional, diverse credentials and licensure types are welcome, must be able to independently practice, and license must be in good standing.
  • Currently holds a New York license or is able to obtain a New York license
  • Minimum of 5 years of post master’s clinical experience as a licensed therapist required.
  • Prior experience working with under-resourced populations required, preferably experience working with adults with T1D.
  • Fluency in English required.
  • Strong interpersonal and communication skills and the ability to work effectively with people of diverse cultures, ages, and economic backgrounds in a culturally competent and sensitive manner.
  • Ability to provide high quality care in a virtual environment.
  • Ability to listen actively, communicate with empathy and gather information in a respectful manner.
  • Ability to use independent judgment and analyze information from multiple sources.
  • Ability to communicate information to medical professionals, clinical employees, coaches, insurance companies, and other community organizations over the phone or virtually.

  • As a part of our care team, this role is expected to get licensed in states where we provide services.
  • For this role, being licensed in New York state or be able to get licensed.
  • An applicant for LCSW who was licensed as a clinical social worker in another jurisdiction and has at least 10 years of practice in the 15 years prior to application.
  • The initial license must have been issued on the basis of meeting requirements similar to those in New York State, including: MSW degree with clinical content; Post-MSW supervised experience in diagnosis, psychotherapy and assessment-based treatment planning, and; ASWB clinical social work examination.
  • An applicant seeking endorsement of a license in Mental Health Counseling issued by another jurisdiction must present evidence of having completed 5 years of licensed practice in the 10 years prior to applying for licensure in New York State.
  • You must have been licensed in the other jurisdiction by meeting the following requirements: holding a graduate degree in counseling or a related field that at the time of completion qualified you for licensure as a Mental Health Counselor in another jurisdiction; completing supervised experience in Mental Health Counseling and psychotherapy that qualified you for initial licensure in the other jurisdiction; and passing an examination acceptable to the New York State Education Department for the practice of Mental Health Counseling.

Responsibilities

  • Interpret full picture of T1-DDAS subscale report and output and create treatment plan alongside the patient for strategies to address these.
  • Document and maintain concise but comprehensive patient plans within medical record.
  • Work collaboratively with the full BCH Care Team, including a range of clinical and non-clinical disciplines, to navigate through the program and coordinate necessary services that may be identified throughout their journey.
  • Act as subject-expert within team on MH and diabetes distress and bring forward education topics (“Edu-corners”) to BCH Care Team.
  • Support and administer patient assessments as needed for patient safety, including but not limited to the PHQ2 and 9.
  • Work collaboratively with patients using an empowerment-based approach.
  • Demonstrate multicultural skills, knowledge and experience, including the influence of cultural issues and providing services with appropriate cultural context.
  • Demonstrate commitment to the welfare of patients, and a deep understanding of the social determinants of health and the needs of under-resourced populations.
  • Monitor behavioral health escalation channel; triage and escalate high-risk cases to ensure timely, appropriate intervention.
