About The Position

Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Licensed Mental Health Counselor (LMHC) for Syracuse Community Health in Syracuse, New York. This is a full-time position in an Outpatient Behavioral Health setting with an Integrated Care model. The role involves providing outpatient behavioral health services, completing assessments, developing treatment plans, monitoring patient progress, and collaborating with a multidisciplinary team. Responsibilities also include crisis intervention, patient advocacy, behavioral health education, and maintaining accurate electronic medical records.

Requirements

  • Active Licensed Mental Health Counselor (LMHC) license in New York.
  • Experience providing individual, family, and group counseling.
  • Ability to perform behavioral health assessments and develop treatment plans.
  • Strong crisis intervention and patient advocacy skills.
  • Experience collaborating within interdisciplinary healthcare teams.
  • Excellent written and verbal communication skills.
  • Electronic medical record documentation experience.

Responsibilities

  • Provide outpatient behavioral health services through individual, family, and group counseling.
  • Complete behavioral health assessments, establish diagnoses, and develop individualized treatment plans.
  • Monitor patient progress and modify treatment plans based on clinical outcomes.
  • Collaborate with primary care providers, psychiatric providers, and interdisciplinary team members to deliver integrated care.
  • Provide crisis intervention, patient advocacy, and behavioral health education.
  • Maintain timely and accurate documentation within the electronic medical record.
