About The Position

The Licensed Medical Social Worker provides medical social services and counseling as part of hospice care for patients and family members during the final stages of illness. Hospice services are delivered in patients’ homes, skilled nursing facilities, assisted living facilities, and adult foster homes. The hospice program also provides intermittent skilled nursing visits, 24-hour on-call nursing services, pain and symptom management, spiritual support, hospice aides and trained volunteers, bereavement support, and medications and equipment as needed.

The position is scheduled for day shifts from 0800 to 1630. Weekend work is not required, but holiday coverage is required. Use of a personal vehicle is required, and the employee must maintain a valid driver’s license and appropriate insurance as mandated by law.

Requirements

• Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure.
• Healthcare Provider Level BLS certification.
• Use of a personal vehicle.
• Maintain a valid driver’s license and appropriate insurance as mandated by law.

Nice To Haves

• Oregon LCSW (Licensed Clinical Social Worker) licensure preferred.
• At least 1 year of experience as a social worker in a hospital or medical setting preferred.
