Requirements

• Current MSW license in the state of Pennsylvania.
• Knowledge of local community resources and support services in Lackawanna, Wyoming, Wayne, and Susquehanna counties.
• Strong communication skills and the ability to work independently and as part of a team.
• Reliable transportation and a valid driver’s license.

Nice To Haves

• Prior experience in home health, hospice, or a similar healthcare setting.
• CPR and First Aid certification.

Responsibilities

• Conduct social work assessments and develop personalized care plans for home health patients.
• Provide counseling and support to patients and families as they adjust to illness, injury, or the challenges of aging.
• Help patients connect with community resources, services, and support networks.
• Collaborate with nurses, therapists, physicians, and other members of the interdisciplinary healthcare team to provide holistic patient care.
• Provide crisis intervention, advocacy, and guidance to patients and their families as needed.
• Maintain thorough and timely documentation of services in compliance with healthcare regulations.
