Human Touch Home Health is seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our growing team serving patients throughout Fairfax, Falls Church, Alexandria, McLean, Vienna, Annandale, Springfield, Burke, Reston, Herndon, Chantilly, Centreville, Leesburg, Ashburn, Sterling, Purcellville, Manassas, Manassas Park, Woodbridge, Gainesville, Haymarket, Dumfries, and surrounding Northern Virginia communities. Requirements

  • Active Virginia Licensed Master Social Worker (LMSW) License Required
  • Master's Degree in Social Work from an Accredited Program
  • Valid Driver's License and Reliable Transportation
  • Strong Assessment, Communication, and Documentation Skills

Nice To Haves

  • Home Health Experience Preferred but Not Required
  • Experience with WellSky
  • Home Health is a plus
  • Passion for Delivering Compassionate, Patient-Centered Care

Responsibilities

  • Complete comprehensive psychosocial assessments
  • Identify emotional, social, environmental, and financial barriers affecting patient care
  • Connect patients and families with community resources and support services
  • Assist with discharge planning and long-term care planning
  • Provide counseling, education, crisis intervention, and emotional support
  • Advocate for patients while coordinating services with community agencies
  • Collaborate with physicians and the interdisciplinary care team
  • Complete timely and accurate clinical documentation using WellSky
