About The Position

The Hospice Licensed Master Social Worker (LMSW) provides comprehensive psychosocial support and services to terminally ill patients and their families. This role involves conducting assessments, offering counseling, connecting patients with community resources, and supporting the interdisciplinary hospice team to ensure holistic care.

Requirements

  • Master’s degree in Social Work (MSW) from an accredited institution.
  • Current licensure as a Master Social Worker (LMSW) in the state of employment.
  • Strong interpersonal and communication skills.
  • Ability to work independently and as part of an interdisciplinary team.
  • Compassionate, empathetic, and able to handle emotionally challenging situations.
  • Knowledge of community resources, healthcare systems, and hospice regulations.
  • Capacity to stand, walk, and sit for extended periods.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Must be able to travel throughout the service area.

Nice To Haves

  • Previous experience in hospice, palliative care, or medical social work preferred.

Responsibilities

  • Conduct initial and ongoing psychosocial assessments of patients and their families.
  • Identify emotional, social, and financial needs and develop care plans to address these needs.
  • Evaluate patients' mental health and coping mechanisms to provide appropriate support and interventions.
  • Provide individual and family counseling to help patients and their loved ones cope with terminal illness and end-of-life issues.
  • Offer grief counseling and support to family members before and after the patient’s death.
  • Facilitate support groups and educational programs for patients and families.
  • Assist patients and families in accessing community resources and services, including financial assistance, legal aid, and support groups.
  • Advocate for patients' rights and needs within the healthcare system and the community.
  • Coordinate with external agencies to ensure patients receive comprehensive care and support.
  • Maintain accurate and timely documentation of patient assessments, care plans, interventions, and outcomes in medical records.
  • Ensure compliance with hospice policies, procedures, and regulatory requirements.
  • Follow infection control guidelines and maintain compliance with HIPAA privacy regulations.
  • Attend mandatory training sessions, in-services, and team meetings.
  • Provide exceptional customer service through compassionate and empathetic care to patients and families.
  • Participate in interdisciplinary team meetings to discuss and review patients’ care plans.
  • Collaborate with nurses, physicians, chaplains, CNAs, and volunteers to provide integrated and patient-centered care.
  • Contribute to the development and implementation of individualized care plans.
  • Performs other related duties as assigned.
