About The Position
A hospital-based healthcare facility is seeking an experienced Licensed Master Social Worker (LMSW) to support inpatient medicine, Med/Surg, and rehabilitation patients. The LMSW will provide psychosocial assessments, supportive counseling, discharge planning, and care coordination in a collaborative clinical environment.
Requirements
- Active NYS LMSW License required
- Master’s Degree in Social Work required
- Minimum 1 year of LMSW experience required
- Hospital experience required
- Strong English communication skills required
Nice To Haves
- Med/Surg or rehab experience preferred
Responsibilities
- Conduct psychosocial assessments and supportive therapy
- Coordinate discharge planning and community referrals
- Work closely with physicians, nurses, and care teams
- Maintain accurate patient documentation
- Support patients and families throughout treatment
