About The Position
We are seeking a Licensed Master Social Worker (LMSW) to provide comprehensive psychosocial support, therapeutic services, and discharge planning within a hospital setting. This role involves working closely with interdisciplinary care teams to ensure patients receive appropriate care during hospitalization and a smooth transition post-discharge.
Requirements
- Master’s Degree in Social Work (MSW) – Required
- Active Licensed Master Social Worker (LMSW) license issued by New York State – Required
- Minimum 1 year of LMSW experience required
- Strong interpersonal and communication skills
- Ability to manage multiple cases and prioritize effectively
- Knowledge of discharge planning and community resources
- Empathy, professionalism, and patient advocacy
Nice To Haves
- Prior hospital or acute care experience preferred
Responsibilities
- Conduct comprehensive psychosocial assessments for patients
- Provide supportive counseling and therapeutic interventions
- Address emotional, social, and environmental factors impacting patient care
- Develop and implement effective discharge plans
- Coordinate referrals to community resources and post-acute services
- Ensure continuity of care following discharge
- Collaborate with physicians, nurses, and multidisciplinary teams
- Support inpatient care teams across medical/surgical and rehabilitation units
- Advocate for patient needs and appropriate care services
