About The Position

The Licensed Marriage and Family Therapist (LMFT) is responsible for providing primary care medical services in a Patient-Centered Health Home context, focusing on access to care. This role involves directly providing behavioral health services and meeting all established productivity expectations. The LMFT will conduct comprehensive clinical assessments, develop individualized treatment plans, and provide therapy sessions utilizing evidence-based modalities. They will facilitate communication and conflict resolution within families and couples, document visits accurately, and attain budgeted visits and clinical care measures. The position requires excellent customer service, courteous interaction with patients and staff, and awareness of patient wait times. The LMFT will provide patients with appropriate educational materials and resources, and promote patient self-management. As an ancillary health team member, they will participate in managing patient care through individual visits and group classes, and assist in maintaining clinical procedures and protocols. Responsibilities also include completing peer review, assisting with performance improvement efforts, staff education, and FHCN promotion. Participation in supervising pre-licensed behavioral health staff may be required. The role involves participating in Network Health Fairs, community events, and site/department meetings. Adherence to the Attendance and Absenteeism Policy is crucial, as is the ability to work at any FHCN location based on business needs. Other duties as assigned.

Requirements

Completion of a Master’s degree program with a recognized major with a minimum cumulative GPA of 2.5; or Successful attainment and maintenance of a valid, active professional license required for the role, issued by the appropriate governing or regulatory body.
Extensive formal training in behavioral health, including an understanding of the application of the theory and practices of the profession to the operations of the organization, usually in the form of a major in Social Work, Psychology, or Marriage and Family Therapy as part of a Master’s degree.
Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
Ability to use advanced functions of Microsoft Excel, such as creating and managing databases, including creating standardized reports or linking multiple worksheets and workbooks.
Ability to develop sophisticated presentations in Microsoft PowerPoint, including embedded objects, transitions, and other elements.
Licensed as a Licensed Marriage and Family Therapist by the state of California.
Job duties require compiling and analyzing information prepared in effective written form, including correspondence, reports, articles, or other documentation.
Duties require preparation and execution of presentations to large groups.
Regularly required to sit and use repetitive hand movements to type and grasp.
Frequently required to stand and walk.
Occasionally lift and/or move up to 20 pounds.

Responsibilities

Consistently behaving courteously when interacting with patients/family members, support staff, and Network staff.
Remaining aware of wait time during the daily workflow.
Providing the patient with educational materials and resources appropriate to the patient’s health literacy needs.
Accountable for patient status as measured by relevant clinical quality measures.
Promoting and participating in daily huddles.
As an ancillary health team member, participating in managing the care of patients/families through individual visits and group classes.
Setting and monitoring self-management plans.
Assisting in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.
Completing peer review responsibilities in a timely manner.
Assisting the patient care team with performance improvement efforts, staff education, and FHCN promotion.
Assisting the patient care team with performance evaluations and quality improvement.
Where relevant, participating in supervising pre-licensed behavioral health staff.
Participating in Network Health Fairs & other community events.
Participating in site & department meetings in a meaningful way.
Meeting Network and credentialing expectations for licensure and employment.
Adhering to the Attendance and Absenteeism Policy.
Ability to present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need.
