About The Position

Our Outpatient Intake Therapist will complete assessments for those seeking service. Requirements

Master’s in Human Services and 1 year of care delivery experience.
Graduate degree from a college or university that is accredited by an agency recognized by the United States Department of Education or the Council for Higher Education Accreditation (CHEA) in a generally recognized clinical discipline in which the degree program includes a clinical practicum

Responsibilities

Complete Intake Assessments
Complete biopsychosocial assessments to determine client’s strengths, needs, and diagnosis
Complete documentation within the electronic health record in congruence with organizational policies and regulatory requirements to ensure quality indicators and productivity requirements are met
Engage with the clients’ natural supports to strategize methods of supporting client goal progress and wellbeing
Collaborate with members of the treatment team, community members, and third-party providers
Attend and participate in clinical, administrative, and community partner meetings
