About The Position

The Licensed Independent Clinical Social Worker (LICSW) will serve as a team advisor, guiding Care Plans, Assessments, and participants' needs within the Adult Day Health setting and their home environment. This individual will act as a participant advocate, assist with all admission functions, and provide supervision support to the Nurse Case Manager. The role involves developing, implementing, and evaluating social service care plans for each assigned participant, coordinating services with other agencies/caregivers, and continuously assessing care plans to ensure participants receive necessary services.

Requirements

  • Must be a LICSW in Massachusetts.
  • Master’s degree in social work or human services from an accredited college.
  • At least one year recent experience working with adults in a professional capacity.
  • Must possess sensitivity to the needs of disabled and low-income older adults.
  • Must be able to work cooperatively as a multi-disciplinary team member.

Nice To Haves

  • Experience in Dementia setting is highly preferred.

Responsibilities

  • Serve as primary advocate for participants and caregivers, coordinating services to support social, emotional, and health-related needs.
  • Develop, implement, and update individualized social service care plans, Behavioral Plans progress notes, and discharge plans as required by regulations.
  • Coordinate services with other agencies, caregivers, and health providers to ensure continuity of care.
  • Conduct family and care planning meetings; participate in interdisciplinary and clinical meetings.
  • Provide counseling, crisis intervention, and behavioral support as needed.
  • Assist participants and families in accessing benefits and community resources when no other advocate is available.
  • Lead or co-lead group discussions, caregiver education, and support sessions.
  • Assist with admission functions including referral management, tours, guest days, and eligibility assessments in collaboration with nursing staff.
  • Work closely with nursing to determine applicant eligibility and in conjunction with nurse make recommendations to Day Program team regarding appropriateness for admission.
  • Participate in Weekly Clinical meetings.
  • Maintain current knowledge of community resources and regulatory requirements.
  • Ensure all care and documentation meets DPH and DMA standards.
  • When applicable Develop, update, and teach Nurse Case Managers the social service plans (admission and progress notes and discharge plans).
  • When applicable provide supervision to Nurse Case Managers.
  • Other program/agency related duties as assigned by supervisor.
