About The Position

The Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Mental Health Counselor (LCMHC) will evaluate, write, monitor, and coordinate behavior management plans with the clinical team in support of and consistent with direct care client care. This role involves working directly with patients when needed, providing quality care in a school-based setting, and ensuring service quality through care coordination and communication with clinical associates, patients, and referral sources. The position also requires participation in performance improvement activities, investigating case situations, presenting information to physicians and the health team, interpreting psychiatric treatment to families, referring patients to community resources, and helping patients understand and follow medical recommendations. Additionally, the role involves supporting, consulting, and training staff on substance abuse and mental health issues, documenting progress in the Electronic Medical Record (EMR), consulting with the clinical team, participating in clinical team meetings, and writing psychosocial reports. The position requires demonstrating knowledge and skills appropriate for the age of patients served, understanding principles of growth and development, and maintaining current practice knowledge. Other duties, work hours, and placement may be assigned.

Requirements

Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Mental Health Counselor (LCMHC)
At least three years of professional clinical experience
Current NH License or license eligible.
Must possess a valid driver’s license.
Strong oral communication skills.
Basic experience/knowledge of computer systems and programs.
Must be organized, self-motivated and have the ability to perform many office tasks.
Ability to perform each essential duty satisfactorily.

Responsibilities

Evaluate, write, monitor, and coordinate behavior management plans with the clinical team.
Works directly in a treatment relationship with patients when needed.
Work with children and provide quality care in a school based setting.
Ensure service quality and participate in care coordination to ensure proper communication between clinical associates, patients, referral sources.
Participate in performance improvement activities.
Investigate case situation and present information to Physicians and other members of the health team on patient's family and social background pertinent to diagnosis and treatment.
Interpret psychiatric treatment to patient's family and help reduce the fear and other attitudes obstructing acceptance of psychiatric care and continuation of treatment.
Refer patient or patient's family to other community resources.
Help the patient to understand, accept, and follow medical recommendations.
Support, consult, and train staff on substance abuse and mental health issues.
Document progress/data/plan of service in the Electronic Medical Record (EMR) utilizing objective, measurable methods.
Consult with clinical team and participate in clinical team meetings.
Write psychosocial reports to be included as part of a patient's Individual Plan of Service.
Ensure report is included in Electronic Medical Record.
Demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served.
Demonstrate knowledge of the principles of growth and development over the lifespan and is able to identify and categorize each patient's age-specific grouping of needs.
Maintain current practice knowledge.
