The Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Mental Health Counselor (LCMHC) will evaluate, write, monitor, and coordinate behavior management plans with the clinical team in support of and consistent with direct care client care. This role involves working directly with patients when needed, providing quality care in a school-based setting, and ensuring service quality through care coordination and communication with clinical associates, patients, and referral sources. The position also requires participation in performance improvement activities, investigating case situations, presenting information to physicians and the health team, interpreting psychiatric treatment to families, referring patients to community resources, and helping patients understand and follow medical recommendations. Additionally, the role involves supporting, consulting, and training staff on substance abuse and mental health issues, documenting progress in the Electronic Medical Record (EMR), consulting with the clinical team, participating in clinical team meetings, and writing psychosocial reports. The position requires demonstrating knowledge and skills appropriate for the age of patients served, understanding principles of growth and development, and maintaining current practice knowledge. Other duties, work hours, and placement may be assigned.
