About The Position

MPAC Healthcare is seeking Licensed Clinicians (LCPC, LMFT, LCSW) to provide high-quality, patient-centered care to seniors. This role offers the autonomy of a private practice without the administrative burden, allowing clinicians to focus on clinical work, patient documentation, and facility correspondence. You will be an on-site provider working with residents and their families, functioning as a lead member of an interdisciplinary team to address emotional, social, and psychological needs. Requirements

  • Master's degree
  • Valid LCSW/LMFT/LCPC license in the state for which you are applying
  • Two years of counseling and therapy experience
  • Ability to work as a member of a team with a great attitude
  • Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches
  • Experience collaborating with interdisciplinary teams
  • Ability to take initiative
  • Effectively communicate in multiple settings with multiple parties (patient, patient’s family, nursing staff, facility social services team, other MPAC providers, etc.)
  • High level of organization to manage patient needs, document effectively and ensure the mental health needs of residents are being met
  • Entrepreneurial spirit
  • Strong medical knowledge
  • Understanding of documentation (CMS, Medicare, Medicaid)
  • Nursing home/hospital facility experience

Responsibilities

  • Provide initial assessments
  • Utilize psychotherapy to address mental health issues
  • Offer ongoing support to residents and their families
  • Manage caseload and schedule
  • Function as an active, lead member on an interdisciplinary team of physicians and nurse practitioners
  • Address residents’ emotional, social, and psychological needs
  • Be responsible for all documentation requirements
  • Keep up-to-date on Medicare Benefit and commercial insurance plans
  • Foster open communication with the facility team and Regional Clinical Manager
