About The Position

MPAC Healthcare is seeking Licensed Clinicians (LCPC, LMFT, LCSW) to provide high-quality care to seniors. This role offers the autonomy of a private practice without the administrative burden, allowing clinicians to focus on patient care and documentation. The position involves working on-site in a skilled nursing facility as part of an interdisciplinary team, addressing residents' emotional, social, and psychological needs. Requirements

• Master's degree.
• Valid LCSW/LMFT/LCPC license in the state of Illinois.
• Two years of counseling and therapy experience.
• Strong medical knowledge.
• Understanding of documentation (CMS, Medicare, Medicaid).
• Experience in nursing home or hospital facility.
• Ability to work as a member of a team with a great attitude.
• Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches.
• Experience collaborating with interdisciplinary teams.
• Ability to foster open communication with the facility team and Regional Clinical Manager.
• Ability to take initiative.
• Effective communication skills in multiple settings with multiple parties.
• High level of organization to manage patient needs and document effectively.

Responsibilities

• Provide initial assessments.
• Utilize psychotherapy to address mental health issues.
• Offer ongoing support to residents and their families.
• Manage caseload and schedule.
• Perform patient documentation.
• Handle facility correspondence.
• Collaborate with physicians, nurse practitioners, and other facility staff on an interdisciplinary team.
• Keep up-to-date on Medicare Benefit and commercial insurance plans.
