About The Position

MPAC Healthcare is seeking Licensed Clinicians (LPC, LMFT, LCSW) to provide high-quality, patient-centered care to seniors. The role offers the autonomy of a private practice without the administrative burden, allowing clinicians to focus on clinical work, documentation, and facility correspondence. As an on-site provider, you will be an integral part of an interdisciplinary team, addressing residents' emotional, social, and psychological needs. This position offers the opportunity to manage your caseload, schedule, and salary, providing initial assessments, utilizing psychotherapy, and offering ongoing support. Responsibilities include all documentation requirements and staying updated on insurance plans.

Requirements

• Master's degree
• Valid LCSW/LMFT/LPC license in the state for which you are applying
• Two years of counseling and therapy experience
• Ability to work as a member of a team with a great attitude
• Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches
• Experience collaborating with interdisciplinary teams, including other MPAC providers, to ensure patients are getting the best care possible
• The ability to foster open communication with the facility team and your Regional Clinical Manager to ensure the success of clinical programs
• Ability to take initiative and effectively communicate in multiple settings with multiple parties (patient, patient’s family, nursing staff, facility social services team, other MPAC providers, etc)
• High level of organization to manage patient needs, document effectively and ensure the mental health needs of residents are being met
• Comfortable with the ever-changing environment of a skilled nursing facility
• Entrepreneurial spirit
• Strong medical knowledge
• Understanding of documentation (CMS, Medicare, Medicaid)
• Nursing home/hospital facility experience

Responsibilities

• Provide initial assessments
• Utilize psychotherapy to address mental health issues
• Offer ongoing support to residents and their families
• Responsible for all documentation requirements
• Keep up-to-date on Medicare Benefit and commercial insurance plans
• Function as an active, lead member on an interdisciplinary team of physicians and nurse practitioners addressing residents’ emotional, social, and psychological needs
• Manage patient needs, document effectively and ensure the mental health needs of residents are being met
