About The Position

MPAC Healthcare is seeking Licensed Clinicians (LPC, LMFT) who are eager to elevate the standard of care for seniors. We are a team of reliable, integrity-driven professionals dedicated to providing high-quality care in the senior care sector. This role offers the autonomy of a private practice without the administrative burden of managing a back office. The position involves working on-site as a clinical provider, addressing the emotional, social, and psychological needs of residents as an active member of an interdisciplinary team. Responsibilities include initial assessments, psychotherapy, ongoing support, patient documentation, and facility correspondence. The role also requires staying updated on Medicare Benefit and commercial insurance plans.

Requirements

• Master’s degree and a valid LMFT/LPC license in the state for which you are applying.
• Two years of counseling and therapy experience.
• Ability to work as a member of a team with a great attitude.
• Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches.
• Experience collaborating with interdisciplinary teams, including other MPAC providers, to ensure patients are getting the best care possible.
• Ability to take initiative and effectively communicate in multiple settings with multiple parties (patient, patient’s family, nursing staff, facility social services team, other MPAC providers, etc).
• High level of organization to manage patient needs, document effectively and ensure the mental health needs of residents are being met.
• Comfortable with the ever-changing environment of a skilled nursing facility.
• Entrepreneurial spirit accompanied by strong medical knowledge.
• Understanding of documentation (CMS, Medicare, Medicaid).
• Nursing home/ hospital facility experience.

Responsibilities

• Function as an active, lead member on an interdisciplinary team of physicians and nurse practitioners addressing residents’ emotional, social, and psychological needs.
• Provide initial assessments, utilize psychotherapy to address mental health issues and offer ongoing support.
• Responsible for all documentation requirements and keeping up-to-date on Medicare Benefit and commercial insurance plans.
• Work within a 40-hour work week, with the majority of time focused on clinical duties and the remainder on patient documentation and facility correspondence.
• Foster open communication with the facility team and your Regional Clinical Manager to ensure the success of clinical programs.
• Manage patient needs, document effectively, and ensure the mental health needs of residents are being met.
