About The Position

MPAC Healthcare is seeking Licensed Clinicians (LCPC, LMFT, LCSW) who are passionate about providing high-quality care to seniors. This role offers the autonomy of a private practice without the administrative burden, allowing clinicians to focus on patient care and documentation. You will be an on-site provider working with residents, their families, and an interdisciplinary team to address emotional, social, and psychological needs. Requirements

  • Master's degree
  • Valid LCSW/LMFT/LCPC license in the state for which you are applying
  • Two years of counseling and therapy experience
  • Ability to work as a member of a team with a great attitude
  • Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches
  • Experience collaborating with interdisciplinary teams, including other MPAC providers, to ensure patients are getting the best care possible
  • Ability to foster open communication with the facility team and your Regional Clinical Manager
  • Ability to take initiative
  • High level of organization to manage patient needs and document effectively

Responsibilities

  • Provide initial assessments
  • Utilize psychotherapy to address mental health issues and offer ongoing support
  • Responsible for all documentation requirements
  • Keep up-to-date on Medicare Benefit and commercial insurance plans
  • Function as an active, lead member on an interdisciplinary team of physicians and nurse practitioners addressing residents’ emotional, social, and psychological needs
  • Manage caseload and schedule
  • Communicate effectively with patients, families, nursing staff, facility social services, and other MPAC providers
  • Ensure the mental health needs of residents are met
