About The Position

MPAC Healthcare is seeking Licensed Clinicians (LPC, LMFT, LICSW) to join their team and help set a new standard for quality care for seniors. This role offers the autonomy of a private practice without the administrative burden of managing a back office. The position involves providing clinical care on-site to residents and their families, functioning as a key member of an interdisciplinary team to address emotional, social, and psychological needs. Clinicians will manage their own caseload and schedule, focusing primarily on clinical duties, patient documentation, and facility correspondence. The role requires providing initial assessments, utilizing psychotherapy, offering ongoing support, and staying updated on Medicare Benefit and commercial insurance plans.

Requirements

• Master’s degree.
• Valid LICSW/LMFT/LPC license in the state for which you are applying.
• Two years of counseling and therapy experience.
• Ability to work as a member of a team with a great attitude.
• Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches.
• Experience collaborating with interdisciplinary teams, including other MPAC providers, to ensure patients are getting the best care possible.
• Ability to foster open communication with the facility team and your Regional Clinical Manager.
• Ability to take initiative and effectively communicate in multiple settings with multiple parties (patient, patient’s family, nursing staff, facility social services team, other MPAC providers, etc).
• High level of organization to manage patient needs, document effectively and ensure the mental health needs of residents are being met.
• Comfortable with the ever-changing environment of a skilled nursing facility.
• Strong medical knowledge.
• Understanding of documentation (CMS, Medicare, Medicaid).
• Nursing home/hospital facility experience.

• Experience in the nursing home space is a plus.

Responsibilities

• Function as an on-site clinical provider working with residents and their families.
• Act as a lead member on an interdisciplinary team of physicians and nurse practitioners addressing residents’ emotional, social, and psychological needs.
• Manage caseload and schedule.
• Provide initial assessments.
• Utilize psychotherapy to address mental health issues and offer ongoing support.
• Be responsible for all documentation requirements.
• Keep up-to-date on Medicare Benefit and commercial insurance plans.
• Focus on clinical duties, patient documentation, and facility correspondence.
