About The Position

MPAC Healthcare is seeking Licensed Clinicians (LCPC, LMFT) to join their team and set a new standard for quality care for seniors. The Licensed Clinician role allows for autonomous functioning, similar to a private practice, without the administrative burden of managing a back office. The position involves clinical focus, patient documentation, and facility correspondence within a 40-hour work week. Clinicians will work on-site with residents and their families, acting as a lead member of an interdisciplinary team to address residents' emotional, social, and psychological needs. Responsibilities include providing initial assessments, utilizing psychotherapy, offering ongoing support, and managing all documentation requirements, including staying updated on Medicare Benefit and commercial insurance plans.

Requirements

• Master’s degree and a valid LMFT/LCPC license in the state for which you are applying.
• Two years of counseling and therapy experience.
• Ability to work as a member of a team with a great attitude.
• Demonstrated knowledge of Mental Health diagnoses and therapeutic approaches.
• Experience collaborating with interdisciplinary teams, including other MPAC providers, to ensure patients are getting the best care possible.
• Ability to take initiative and effectively communicate in multiple settings with multiple parties (patient, patient’s family, nursing staff, facility social services team, other MPAC providers, etc).
• High level of organization to manage patient needs, document effectively and ensure the mental health needs of residents are being met.
• Comfortable with the ever-changing environment of a skilled nursing facility.
• Knowledge of the Medicare Benefit and commercial insurance plans.
• Experience in the healthcare setting.

Responsibilities

• Function as an active, lead member on an interdisciplinary team of physicians and nurse practitioners addressing residents’ emotional, social, and psychological needs.
• Provide initial assessments, utilize psychotherapy to address mental health issues and offer ongoing support.
• Responsible for all documentation requirements and keeping up-to-date on Medicare Benefit and commercial insurance plans.
• Manage patient needs, document effectively and ensure the mental health needs of residents are being met.
• Foster open communication with the facility team and your Regional Clinical Manager to ensure the success of clinical programs.
