About The Position

SMG Family Medicine New Town is seeking a Licensed Clinical Social Worker (LCSW) to join their collaborative outpatient care team in Williamsburg, VA. This role involves participating as an integral part of a multidisciplinary team to establish comprehensive and integrated plans for each patient. The LCSW will be involved in both the delivery of clinical services and referrals to appropriate community resources and other professionals. The position assists patients and their families in meeting psychosocial needs that may predispose illness or interfere with achieving an optimal level of care. Responsibilities include assessing social, psychological, cultural, environmental, and financial situations, as well as disposition needs. The LCSW provides individualized care addressing physical, psychological, socio-cultural, spiritual, and cognitive needs, and performs patient assessments through systematic data collection and review. They also assess patient/family learning needs, plan and provide education, and evaluate its effectiveness. The role consistently includes the patient/family in care planning and outcome evaluation, demonstrating effective use of the social work process. Care plans are based on assessment, patient progress is evaluated, and care is revised to attain expected outcomes. The LCSW identifies and responds to ethical issues, provides care in a non-judgmental manner respecting diversity and patient rights, and integrates age-specific care principles. Clinical intervention and consultation are provided as a member of the multidisciplinary team. Crisis intervention services are offered, facilitating referrals to other professionals and community resources as needed. Individual, family, and group counseling are provided as required. The LCSW regularly participates in multidisciplinary team meetings, including rounds, and collaborates on developing and implementing patient care plans. Progress is monitored, outcomes evaluated, and plans modified as necessary. Bereavement counseling is offered to patients, families, and staff. Consultation to other team members regarding identified problems or issues is regularly provided. Patients at high risk for non-compliance are identified, and a comprehensive plan is developed and implemented.

Requirements

Minimum of 2 years of social work experience required
Master's degree in social work required
Licensed Clinical Social Worker (LCSW) required

Demonstrates effective use of the social work process to deliver individualized care to patients.
Plans care based on the assessment, evaluates patient progress and revises care to attain expected outcomes.
Identifies and responds appropriately to ethical issues in patient care; provides care in a non-judgmental manner that respects patient diversity and acknowledges patients' rights.
Integrates principles of age-specific care into all aspects of practices.
Provides clinical intervention, consultation as a member of the multidisciplinary team.
Provides crisis intervention services and facilitates referral to other psychiatric/psychological professionals and community resources/agencies as needed.
Provides individual, family and group counseling as needed.
Regularly participates as a member of the multidisciplinary team including attendance at rounds.
In concert with other members of the team, develops and implements a patient plan of care to address identified psychosocial problems or needs.
Monitors the progress of the plan, evaluates outcomes and modifies the plan as needed.
Provides bereavement counseling for patients, families, and members of the staff.
Regularly provides consultation to the other members of the professional team related to identified problems or issues.
Identifies patients who are at high risk for non-compliance or failure to adhere to the prescribed regimen and develops and implements a comprehensive plan.
