About The Position

The Licensed Clinical Social Worker (LCSW) is responsible for the provision of primary care medical services in a Patient Centered Health Home context, with a focus on access to care.

Requirements

  • Equivalent of extensive formal training in behavioral health, including an understanding of the application of the theory and practices of the profession to the operations of the organization, usually in the form of a major in Social Work as part of a Master’s Degree.
  • Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
  • Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.
  • Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions and other elements.
  • Licensed as Licensed Clinical Social Worker by the state of California.
  • Ability to compile and analyze information prepared in effective written form, including correspondence, reports, articles or other documentation.
  • Ability to prepare and execute presentations to large groups.

Responsibilities

  • Direct provision of behavioral health services meeting all established productivity expectations, including examination of relevant symptoms and history to determine a diagnosis, accurate documentation utilizing appropriate templates, and recording relevant data to attain a diagnosis.
  • Attaining budgeted visits and meeting clinical care measures.
  • Locking records in the 72-hour window.
  • Providing excellent customer service and care, behaving with courtesy when interacting with patients/family members, support staff, and Network staff.
  • Remaining aware of wait times during the daily workflow.
  • Providing patients with educational materials and resources appropriate to their health literacy needs.
  • Accountable for patient status as measured by relevant clinical quality measures.
  • Promoting and participating in daily huddles.
  • As an ancillary health team member, participating in managing the care of patients/families through individual visits and group classes.
  • Setting and monitoring self-management plans.
  • Assisting in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.
  • Completing chart and peer review responsibility in a timely manner.
  • Assisting the patient care team with performance improvement efforts, staff education, and FHCN promotion.
  • Assisting the patient care team with performance evaluations and quality improvement.
  • Where relevant, participating in supervision of pre-licensed behavioral health staff.
  • Participating in Network Health Fairs & other community events.
  • Participating in site & provider meetings in a meaningful way.
  • Meeting Network and credentialing expectations for licensure, and employment.
  • Performing other duties as assigned.
