Licensed Clinical Social Worker
About The Position
Cottage Hospital is seeking a compassionate and skilled Social Worker to support patients through comprehensive psychosocial care. In this role, you will conduct thorough assessments to identify patient needs, establish diagnoses, and develop individualized treatment plans that support overall well-being. You will serve as an active member of the interdisciplinary care team, providing emotional support and mental health counseling to both individuals and groups. Responsibilities include crisis intervention, care coordination, and ensuring patients and their families have access to appropriate resources and support systems. This role requires strong knowledge of community-based social, medical, and psychiatric services. You will manage referrals to outside providers and agencies, helping patients navigate care options and ensuring continuity of treatment. Maintaining up-to-date resource materials and building relationships with community partners will be essential to success.
