About The Position

They offer integrated primary medical care, behavioral health, pharmacy, and dental services. The Licensed Clinical Social Worker at Wilson Community Health Center is integral to their progressive approach to holistic healthcare, working within a team to provide integrated behavioral health consultation to children, adolescents, adults, and families. This role aims to improve psychosocial functioning by addressing mental health and substance abuse conditions, health behaviors, life stressors, stress-related physical symptoms, and healthcare utilization patterns. The position also supports primary care physicians in developing care management processes, including guidelines, disease management, case management, and patient education.

Requirements

  • Master's degree in social work, psychology, or related Behavioral Health field (from regionally accredited and approved institutions).
  • Demonstrated working knowledge of brief, evidence-based treatment modalities.
  • Ability to manage the pace of primary care and work with an interdisciplinary team.
  • Intermediate level computer skills.
  • Strong interpersonal skills including the ability to demonstrate empathy.
  • Effective communication skills.
  • Analytical skills.
  • BLS Certification.
  • Valid North Carolina Driver's License.

Nice To Haves

  • Minimum one-year field experience in a primary care setting preferred, but not required.
  • Fully licensed LCSW, licensed to practice in the state of North Carolina, is preferred.
  • Associate Licensed LCSW will be considered.
  • Recent graduate will be considered.

Responsibilities

  • Assess the clinical status of patients referred by primary care providers and outside referral sources to develop a patient-focused plan of care for substance use disorder and/or mental health treatment.
  • Incorporate chronic disease management into overall treatment goals.
  • Use evidence-based assessment screening tools with patients to assist with identifying concerns and outcome measurements.
  • Consult with medical providers as necessary, and refer cases to specialty, case management, and other behavioral health/substance use providers as appropriate.
  • Work with the primary care team to treat and manage patients with chronic mental health problems and/or substance use disorders efficiently and effectively to improve clinical outcomes.
  • Educate staff via in-services on related behavioral health topics when needed.
  • Teach treatment enhancement techniques using non-stigmatizing language and empathy to patients, families, and staff.
  • Accurately record patients' past medical history, social and medication history, records therapy provided, and referrals in the medical record, according to established format.
  • Complete documentation in a timely manner according to protocol.
