About The Position

Coordinate the interdisciplinary plan of care across the continuum from admission to discharge while integrating patient and family preferences. Monitor patient care through assessments, evaluation, and record reviews while serving as a resource for documentation, service appropriateness, and post-discharge options. Advocate for necessary resources and barrier removal while maintaining ongoing dialogue with supervisors and providers to execute care plans. Provide clinical oversight of other associates as needed.

Requirements

Clinical Counselor obtained prior to hire date or job transfer date.
Counselor specializing in Clinical Practice obtained prior to hire date or job transfer date.
Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date.
Master's degree in Social Work required.
Able to commute, if needed, to the Chicago, IL area

Nice To Haves

Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
Case Manager credentialed from the American Case Management Association (ACMA) preferred.
3 years of experience preferred
Hospital care management experience needed
Previous Behavioral Health experience desired
