About The Position

The LCSW is responsible for assessing and coordinating the clinical management of patients in specific case groups for an episode of care. The LCSW is responsible for developing and meeting desired patient care outcomes for his/her caseload based on assessment of patient care needs and as ordered by the physician, within an appropriate length of stay and appropriate use of resources. The LCSW responds to complex patient care needs from admission to discharge, works productively and cooperatively in collaboration with the physician and other members of the interdisciplinary team.

Requirements

  • Masters of Science in Social Work
  • Current LCSW for the State of Illinois
  • 3-5 yrs Experience as an LCSW
  • Current BLS License
  • Experience using an EHR system, Meditech Expanse preferred
  • CPI and Team Restraint required within 30 days of hire

Responsibilities

  • Completes a comprehensive psychosocial assessment on all patients within 72 hours of admission.
  • Assess medical necessity and intensity of services using the Milliman and Interqual Criteria.
  • Conduct insurance notification, certification and concurrent reviews on all assigned patients.
  • Coordinates individual staffing/treatment planning meetings within 72 hours of admission to develop individual treatment plans, monitor the effectiveness of care.
  • Arrange aftercare services to address identified needs.
  • Confirm referral acceptance 24 hours prior to discharge.
  • Keeps guardian abreast of patient progress and discharge planning
  • Provide follow-up calls to discharge patients to monitor compliance with aftercare plan.
  • Provide individual, family and group therapy.
  • Responsible for the content of education and counseling programs for both individual patients or groups of patients and serves as liaison to community, individuals and groups as appropriate.
  • Review, authenticate, and electronically sign therapist progress notes in Medi‑Tech Expanse in accordance with Joint Commission documentation standards, hospital policy, and regulatory requirements.
  • Demonstrate the ability to prioritize workload efficiently and accept/respond to change in a manner that promotes and supports departmental/hospital goals and objectives.
  • Maintain accurate and timely documentation.
  • Understands and adheres to Loretto Hospital’s compliance standards as they appear in the Compliance Policy, Code of Conduct and Conflict of Interest Policy.
  • Keeps abreast of all pertinent federal, state and Hospital regulations, laws, and policies as they presently exist and as they change or are modified.
  • Ensures that the staff are trained and evaluated on their knowledge of and adherence to compliance policies and procedures specific to their jobs.
