About The Position
Mobile Medical is seeking a compassionate Licensed Clinical Social Worker (LCSW) to provide behavioral health support, counseling, care coordination, and resource management to patients throughout Duval County. This role offers a unique opportunity to build meaningful relationships with patients while helping address the social, emotional, and behavioral barriers that impact their health and quality of life.
Requirements
- Master's Degree in Social Work (MSW)
- Active Florida Licensed Clinical Social Worker (LCSW) License
- Minimum of 1 year of clinical counseling experience
- Strong communication and relationship-building skills
- Valid Florida Driver's License and reliable transportation

Responsibilities
- Conduct psychosocial assessments and behavioral health evaluations
- Provide individual counseling and supportive therapy
- Develop and implement treatment plans
- Identify barriers to care and connect patients with community resources
- Assist patients with social, emotional, financial, and behavioral health needs
- Coordinate care with providers and interdisciplinary teams
- Educate patients and families regarding available support services
- Maintain accurate and timely documentation
- Ensure compliance with Medicare and company documentation standards
- Support continuity of care and positive patient outcomes
