About The Position
Compass Palliative Care is seeking a compassionate, licensed Clinical Social Worker to provide in-person support to patients in Assisted Living Facilities (ALFs) in Riverside, California. The role is part-time, scheduled one day per week for 8–10 hours per day, with pay of $40–$45 per hour.
Requirements
- Active, unrestricted California LCSW license.
- Strong communication and organizational skills.
- Ability to work independently and travel to in-person visits.
Nice To Haves
- Experience with older adults, medically complex patients, or palliative care.
Responsibilities
- Provide social work assessments and support tailored to patients’ needs.
- Help patients and families navigate care resources and address psychosocial concerns.
- Coordinate with facility staff and the healthcare team.
- Maintain timely and accurate clinical documentation.
