About The Position

Within Hill Physicians’ care coordination framework, the Licensed Clinical Social Worker provides telephonic psychosocial assessments and interventions to help patients and caregivers manage care transitions and cope with illness, diagnosis, treatment, and life situations, including emotional, mental, and substance abuse disorders.

Requirements

• LCSW.
• Two to four years of social work experience within the last five years; MSW fieldwork is not included.

Nice To Haves

• Social work experience in a managed care environment.

Responsibilities

• Coordinate appropriate services, including Home Health or Hospice care and other levels of care such as SNF, Board and Care, and caregivers at home.
• Educate patients, family members, and caregivers about adapting to the patient’s diagnosis, illness, treatment, or life situation.
• Participate in multidisciplinary health care teams.
• Assist with advance care planning and completion of Advance Directives and POLST.
• Coordinate symptom management and/or end-of-life services.
• Assess and refer patients for Medi-Cal and/or other insurance eligibility based on new entitlements, and refer for SSI and/or SSDI as needed or within scope of care definitions.
