About The Position

Partners in Health is seeking a Licensed Clinical Social Worker to provide exceptional behavioral health interventions within an integrated care setting. This role involves working with a diverse population facing various mental health challenges, including substance abuse, and collaborating closely with primary care medical providers to deliver whole-person care. The position requires coordinating care with community providers, setting and monitoring clinical goals, participating in team meetings, and offering both in-person and telehealth treatment. The ideal candidate will be willing to explore new clinical interventions and maintain timely documentation. This position is based out of the Westminster office and requires travel between all four clinic locations.

Requirements

  • Master's Degree (from accredited university/college) in Social Work
  • License required: LCSW
  • Self-motivated and the ability to work independently as well as within a team
  • Experience coordinating care within the community to wrap around services that may not be available within the primary care setting.
  • Ability to assess and appropriately manage crisis situations that require risk evaluation and management.

Responsibilities

  • Provide exceptional behavioral health interventions, within an integrated care setting, utilizing therapeutic intervention models (brief solution focused, CBT, MI, ACT etc.)
  • Work effectively and collaboratively with the primary care medical providers to deliver whole person care
  • Coordinate care with other community providers and provide linkage and referrals to other services as needed.
  • Set and monitor achievable clinical goals and outcomes
  • Participation in provider team meetings.
  • Ability to provide both in person and telehealth treatment.
  • Willingness to explore other clinical interventions that may benefit the patients of the practice (i.e., clinical groups or other modes of providing care as deemed appropriate and beneficial)
  • Timely documentation
  • Other duties as assigned
