About The Position

This position works collaboratively with medical, dental, and behavioral health providers to support whole-person care and improve patient outcomes through counseling, care coordination, crisis intervention, and recovery support services. The LCSW will deliver evidence-based, trauma-informed care to individuals experiencing substance use disorders and co-occurring mental health conditions.

Requirements

  • Master’s Degree in Social Work (MSW) from an accredited program required
  • Current Illinois Licensed Clinical Social Worker (LCSW) license required

Nice To Haves

  • CADC or experience in substance use treatment preferred
  • Minimum of one (1) year experience in behavioral health or substance use counseling preferred
  • Experience working with co-occurring disorders and underserved populations strongly preferred
  • Group counseling experience preferred
  • Strong understanding of substance use disorders, recovery models, and behavioral health interventions
  • Excellent communication, documentation, and interpersonal skills
  • Ability to work independently and collaboratively within an integrated care team
  • Knowledge of community resources and referral systems
  • Strong organizational and time-management skills
  • Ability to maintain professional boundaries and confidentiality

Responsibilities

  • Conduct comprehensive biopsychosocial and substance use assessments
  • Evaluate patients for co-occurring mental health and substance use disorders
  • Determine appropriate levels of care and develop individualized treatment plans
  • Provide screening interviews and referrals for additional behavioral health or community services as needed
  • Provide individual, family, and group counseling services related to substance use and recovery
  • Utilize evidence-based therapeutic approaches including: Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), Trauma-Informed Care, Relapse Prevention Strategies
  • Support patients in identifying barriers to recovery and developing healthy coping skills
  • Monitor patient progress and modify treatment plans as clinically appropriate
  • Coordinate referrals to inpatient, outpatient, detoxification, medication-assisted treatment (MAT), and community recovery resources
  • Collaborate with medical providers, case managers, behavioral health staff, and outside agencies to ensure continuity of care
  • Advocate for patients and assist with access to transportation, support services, and recovery resources
  • Participate in crisis intervention and safety planning when necessary
  • Maintain accurate, timely, and confidential clinical documentation within the electronic health record (EHR)
  • Ensure compliance with HIPAA, 42 CFR Part 2, HRSA, state licensing regulations, and organizational policies
  • Participate in staff meetings, trainings, and quality improvement initiatives
