The Licensed Clinical Social Worker (LCSW) provides advanced psychosocial support, counseling, and care coordination services to patients and families in inpatient, outpatient, and ambulatory care settings. As a key member of an interdisciplinary team, the LCSW delivers evidence-based interventions, facilitates access to healthcare and community resources, and supports patients in navigating complex psychosocial challenges to improve overall health and well-being. This role is critical in addressing social drivers of health, developing individualized care plans, and contributing to effective discharge planning and crisis intervention. The LCSW also plays an integral part in program development, quality improvement initiatives, and mentoring of social work team members. The LCSW collaborates closely with physicians, nurses, case managers, and allied health professionals to ensure a holistic, patient-centered approach to care. This position requires independent clinical practice within the scope of licensure, as defined by the Texas State Board of Social Work Examiners (TSBSWE), and adheres to high standards of ethical and professional conduct.
