About The Position

Silver Lining Behavioral Health is seeking a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Psychologist to provide care for the aging population in various settings, including telehealth, senior living facilities, healthcare settings, and in-home visits. The therapist will collaborate with Silver Lining’s outpatient counseling and psychiatric nurses to offer psychotherapy services to older adults experiencing behavioral health issues. This role involves coordinating client care with a care liaison, assisting with insurance pre-authorization, and managing caseloads. The therapist will build relationships with patients, their families, and caregivers, guiding them through the challenges of late-life mental illnesses and dementia. Responsibilities include conducting behavioral health diagnostics, crisis evaluations, and utilizing standardized assessment tools. The position also involves providing ongoing psychotherapy in individual and group settings, conducting in-home and virtual support services, and offering case management to patients and caregivers.

Requirements

  • Independent LCSW or LCPC in the state of Illinois
  • At least one year providing direct client care.
  • Experience of working with older adults
  • Self-motivated and independent
  • Familiarity with community mental health resources
  • Must possess an active license to practice counseling or social work in the State of Illinois: LCPC, LCSW
  • Experience in delivering high-quality, client-centered care
  • Able to document and navigate EHR independently
  • Ability to maintain strict client confidentiality
  • Ability to effectively communicate and interact with individuals involved in all aspects of programming – children, parents, school faculty and staff, grantors, funders, partner agencies, members of the community and Silver Lining team members
  • Proficient computer skills, including working knowledge of Microsoft Office Suite
  • Ability to work effectively on multidisciplinary team
  • Language Skills: Ability to read, analyze and interpret general and professional documents. Ability to write routine reports and correspondence. Ability to effectively present information in one on one and small group situations
  • Must possess the ability to make independent decisions when circumstances warrant such action
  • Ability to foster teamwork; train and direct the work of team members and colleagues
  • Excellent organizational skills, including the ability to work productively and make independent decisions

Responsibilities

  • Provide counseling and psychotherapeutic services consistent with licensure and training to clients in various clinical settings.
  • Assess, diagnose, and treat adults, adolescents, and children experiencing symptoms of behavioral health disorders.
  • Collaborate with counselors, social workers, or other treatment team members to coordinate client care.
  • Complete medical records, including assessment, treatment plan, progress notes, outcome measures, and billing entries in a timely and accurate manner.
  • Counsel clients during appointments as necessary.
  • Design and participate in the development of individualized treatment plans, incorporating best and leading practices into treatment interventions.
  • Collaborate with guardians, relatives, and significant others on client’s conditions and treatment when clinically appropriate.
  • Prepare and submit assessments, progress notes, reports, and summaries to entities/agencies (funders, insurance companies, etc.) as required.
  • Return calls and emails promptly (within 24 hours of receipt or sooner if clinically necessary).
  • Make independent decisions based on protocols developed utilizing national guidelines for treatment interventions.
  • Maintain customer relations with all agency staff and clients.
  • Record all services performed for accurate billing in compliance with all local, state, and federal laws, as well as payor rules and regulations, submitting billing documents in a timely fashion.
  • Look for ways to improve and promote quality.
  • Provide teaching and education to clinical staff, if requested.
  • Follow all procedures and protocols as per Silver Lining’s policy, federal and state regulations, and standard practice.
  • Develop alliances with patients, their families, and caregivers.
  • Guide geriatric clients and families through the challenges of late-life mental illnesses and the journey of dementia.
  • Conduct behavioral health diagnostics and crisis evaluations.
  • Utilize standardized cognitive, behavioral, and mental health assessment tools.
  • Provide ongoing psychotherapy in individual and group settings for patients and caregivers.
  • Conduct in-home, tele-presentation/virtual, and telephone support services for patients and their families, caregivers.
  • Provide case management to patients and caregivers.
