About The Position
The Licensed Clinical Social Worker (LCSW) leads the assessment and coordination of social services for residents in our Skilled Nursing, Assisted Living, and Independent Living neighborhoods. This role promotes Resident Centered Care by addressing psychosocial needs, supporting families and ensuring compliance with all federal, state and local regulations while upholding Sunrise’s Mission and Principles of Service.

Responsibilities
- Assess residents’ psychosocial needs and document care plans per regulatory requirements
- Lead and participate in family and care plan conferences
- Provide counseling and one-on-one support to residents and families
- Coordinate admissions, discharges and room transfers
- Complete and support MDS and care planning processes
- Act as liaison with community agencies and external resources
- Support Resident Council and coordinate transportation as needed
- Ensure compliance with social services regulations and Sunrise standards
- Manage departmental operations, staffing, scheduling and engagement initiatives
- Partner with leadership on budget management and financial oversight
