Requirements

Fully licensed as an LCSW in the State of Indiana.
Experienced in working with adult, and/or child and adolescent populations.
Must be a W2 employee.

Responsibilities

Provide evidence-based, medically driven treatment services for children, adolescents and adults suffering from a variety of mental health issues in an outpatient care setting.
Conduct patient assessments and develop treatment plans.
Deliver therapy services both in-person and through telemedicine.
Collaborate with other healthcare professionals as needed.
Maintain accurate and timely patient records.
