The Licensed Clinical Social Worker (LCSW) is responsible for making clinical decisions and exercising judgment to ensure the best possible care is provided within available resources. This role involves providing counseling services, particularly for individual and family issues that impact patient health. The LCSW will work within a primary care model, offering brief therapy and facilitating seamless transitions to community resources. A key aspect of this role is maintaining knowledge of community resources, especially in crisis services, mental health, and long-term care. The LCSW will also assist referring physicians and healthcare providers with referrals to secondary and tertiary health care centers. Additionally, the position requires expertise in group work, including cognitive behavior therapy and mind over mood techniques. The ideal candidate will be enthusiastic, passionate, and self-driven, participating in data management and quality improvement initiatives to measure patient outcomes. This includes communicating results in an understandable format and identifying patterns for improvement. The LCSW will act as an advocate for patients and their families, liaise with physicians and other team members, and teach family medicine residents and other healthcare professional students about the social worker's role and psychosocial issues. Effective use of social work standards of practice and the therapy process for patient care assessment, planning, implementation, and evaluation is essential. Maintaining continuing education, collaborating with the healthcare team, and serving as a professional role model are also key responsibilities. Confidentiality in the workplace is paramount.
