About The Position

Reports to the Social Work Manager, the Licensed Clinical Social Worker (LCSW) is a member of the Social Work Department located in the primary care offices to provide care management services to their patients. The LCSW is primarily responsible for face-to-face comprehensive biopsychosocial assessments, collaboration with patient’s primary care physician to determine diagnosis and treatment through brief therapy and continued follow-up with patients throughout the duration of the therapy process. The nature of this practice will consist of working with adult and geriatric patients who present with emotional disorders and mental illness in a primary care setting. Patients who present to their primary care physicians will be assessed, evaluated, collaboration with patient’s primary care physician to determine diagnosis and treated by the LCSW. A LCSW is a member of the Social Work Department and works closely with the entire care team to provide optimal services to the patient. The LCSW demonstrates respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background.

Requirements

  • MSW from accredited school by the Council on Social Work
  • 2+ years clinical social work experience in mental health.
  • KY - Licensed Clinical Social Worker (LCSW)

Responsibilities

  • Provides clinical social work, which focuses on the assessment, collaboration with patient’s primary care visit to determine diagnosis and treatment of an emotional and mental illness as related to the total health of the patient.
  • Develops comprehensive psychosocial assessments, which examine the current problem(s), as well as relevant bio-psychosocial and environmental factors in order to establish an appropriate care plan.
  • Initiate contact with patient/family in the Physician’s office or by phone within 1 business day from referral date.
  • Communicates appropriate and applicable information on chart in accordance with HIPPA guidelines.
  • Creates a positive therapeutic relationship with client that leads to correction of the dysfunction.
  • Provides a wide range of treatment modalities Based on assessment which may consist of: short-term CBT, solution-focused, motivational interviewing; problem solving; crisis intervention; mindfulness; trauma-informed; behavior activation; acceptance and commitment, person-centered or any number of effective evidence-based options to stabilize and regulate the current dysfunction.
  • Identifies social determinants that impair/impact the patient’s inability to pursue treatment or barriers that inhibit patient’s on-going health issues and works closely with the Social Work Department to refer patients for case management needs.
  • Treatment planning that includes development, implementation and modification of the plan
  • Evaluation of progress on patients care plans
  • Termination of the treatment process or referral to higher level of care when necessary
  • Initiates contact with primary care patients via phone with primary care patients who have been discharged from the St. Elizabeth emergency room after a mental health evaluation for further mental health needs for therapy.
  • Documents all visits and phone contacts accurately and in a timely manner and Identifies and documents all interventions and case closure plans in EPIC.
  • Initiates patient/family education, counseling and other social work interventions to enhance problem solving and coping.
  • Acts as a liaison between patient/family and all members of the healthcare team.
  • Continuously seeks to increase the quality and value of services provided to our patients/family and health care teams.
  • Builds rapport with patients/family and healthcare teams.
  • Expands and/or strengthens patient’s social supports.
  • Serves as consultant to the interdisciplinary team regarding psychosocial components, procedures or services to promote greater understanding of the patient.
  • Communicates effectively with co-workers and healthcare team to ensure continuity of care and service.
  • Resolve issues or problems in a direct, open and constructive manner.
  • Honors the dignity of every individual in all interactions.
  • Integrates knowledge, skill and experience to continuously improve self and the quality of patient care.
  • Values feedback as an opportunity to learn.
  • Maintains skills and knowledge as appropriate to the medical social work role.
  • Participates in continuing education to fulfill licensure requirements.
  • Assists in the process to evaluate patient outcomes, financial and clinical results that can be tracked to measure success of the Social Service Department.
  • Other duties and responsibilities as assigned are completed on a timely, throughout and accurate basis.
