About The Position
Make a meaningful impact by supporting patients and families through work as a Social Worker while helping improve continuity and quality of care in a close-knit community. This is a full-time, permanent position with a Monday-Friday schedule, located near Gallup, NM. The role involves providing direct support and case management services to assigned patients and families, coordinating resources and services to support positive patient outcomes, and advocating for patients while helping navigate social, emotional, and community-based needs.
Requirements
- Current Licensed Social Worker (LSW) license required
- LICSW candidates are highly preferred
- Hospital or healthcare experience preferred
