PER DIEM LICENSED BEHAVIORAL HEALTH THERAPIST or LICENSED CLINICAL SOCIAL WORKER
About The Position
Coulee Medical Center is searching for a Per Diem Licensed Behavioral Health Therapist or Licensed Clinical Social Worker, hybrid schedule options available! This position plays a vital role in Coulee Medical Center’s collaborative care team, consisting of the primary care provider, a psychiatric consultant, the patient, and a larger primary care group. The clinician is responsible for supporting and coordinating the behavioral and physical health care of patients on an assigned patient caseload with the patient’s primary care provider and, when appropriate, other behavioral health providers. The Per Diem Licensed Behavioral Health Therapist or Licensed Clinical Social Worker supports the organization by being an active member of the medical team, and carries out these duties in a manner consistent with the mission and vision of Coulee Medical Center.
Requirements
- A minimum of an associate pursuing master’s level training and licensure in any of the following: Licensed Behavioral Health Counselor/Professional Counselor, Licensed Marriage and Family Therapist, Licensed Social Worker, Licensed Psychologist.
Responsibilities
- Supporting and coordinating the behavioral and physical health care of patients on an assigned patient caseload with the patient’s primary care provider and, when appropriate, other behavioral health providers.
- Being an active member of the medical team.
