About The Position

At Father Joe’s Villages, integrated healthcare is central to our mission to end homelessness and advance health equity in San Diego. Through the Village Health Center, a Federally Qualified Health Center (FQHC), we provide trauma-informed, whole-person care that integrates behavioral health, primary care, and social services. Our behavioral health team supports individuals experiencing complex medical, psychiatric, and social needs, including homelessness, serious mental illness, substance use disorders, and co-occurring conditions. This is a highly integrated care environment where clinical collaboration, independent judgment, and real-time decision-making are essential to effective care delivery. The Behavioral Health Clinician II is a licensed independent clinician responsible for providing advanced behavioral health services within a multidisciplinary FQHC setting. This role serves as a key clinical partner to primary care and specialty providers, delivering assessment, psychotherapy, crisis intervention, and consultation services for individuals with complex and often high-acuity needs. Clinicians in this role are expected to exercise strong clinical judgment, manage their caseload independently, and contribute meaningfully to integrated care planning and team-based decision-making.

Requirements

  • Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or a closely related behavioral health field from an accredited program
  • Current, active, and unrestricted California clinical license in good standing (LCSW, LMFT, or LPCC)
  • Minimum of one (1) year post-licensure clinical experience, or two (2) years post-graduate experience providing psychotherapy in an outpatient, community mental health, or integrated care setting
  • Experience working with underserved, low-income, and/or medically and psychosocially complex populations, including individuals experiencing homelessness
  • Working knowledge of DSM-5 diagnosis, co-occurring disorders, trauma-informed care, and crisis intervention principles
  • Current CPR certification, or ability to obtain upon hire

  • Experience in FQHC, integrated primary care, or multidisciplinary healthcare settings
  • Experience managing complex clinical presentations, including co-occurring disorders and high-risk clients
  • Comfort working independently in high-volume clinical environments while collaborating closely with medical teams
  • Ability to deliver brief, focused, evidence-based interventions in time-limited settings
  • Strong clinical judgment with ability to navigate ambiguity and shifting clinical priorities
  • Experience providing consultation to medical providers or non-behavioral health staff
  • Skilled in building rapport quickly with diverse populations in high-stress or low-trust environments
  • Bilingual (English/Spanish) strongly preferred
  • Strong clinical expertise in DSM-5 diagnosis, differential diagnosis, and complex behavioral health presentations
  • Experience with co-occurring disorders, trauma-informed care, harm reduction, and recovery-oriented practice
  • Ability to conduct risk assessment, crisis intervention, and stabilization planning in high-acuity situations
  • Ability to independently determine clinical acuity and appropriate level of care, including escalation when needed
  • Experience working effectively in integrated care teams, with ability to translate behavioral health concepts for medical providers and staff
  • Strong interpersonal skills with ability to build trust quickly in diverse, underserved, and low-trust populations
  • High cultural humility, professional integrity, and emotional resilience in complex clinical environments

Responsibilities

  • Conduct comprehensive diagnostic evaluations, including complex psychosocial and differential assessments in high-acuity and medically complex populations
  • Provide short-term, evidence-based psychotherapy and group interventions tailored to integrated care settings
  • Lead crisis assessment, stabilization, and clinical risk management, including safety planning and triage decisions
  • Serve as a key clinical partner to primary care and specialty medical providers within an integrated FQHC care model
  • Contribute to interdisciplinary treatment planning for patients with complex behavioral health, medical, and social needs
  • Provide real-time behavioral health consultation to medical teams to support shared clinical decision-making
  • Maintain and independently manage a diverse clinical caseload with appropriate clinical judgment regarding intensity, modality, and level of care
  • Ensure continuity of care through effective engagement, follow-up, and coordination across services and systems
  • Deliver timely, high-quality clinical documentation aligned with FQHC, regulatory, and medical necessity standards
  • Participate in peer consultation, case review, and quality improvement efforts to strengthen clinical practice and care outcomes
  • Maintain licensure, continuing education, and compliance with all professional and organizational requirements
