About The Position

We are seeking a Licensed Alcohol and Drug Counselor (LADC) to join our Youth Assertive Community Treatment team. The purpose of this position is to provide mental health and case management services to youth and families referred to Northern Pines Mental Health Center. Essential Functions include: Establish professional relationships with collateral contacts Provide substance use and skills-based treatment services Provide ongoing assessment of clients’ substance use symptoms and clients’ response to treatment. Make appropriate changes in treatment plans to ensure immediate and appropriate interventions are provided in response to changes in behavior which put clients at risk Consult with community agencies and families to maintain coordination in the treatment process Complete an IRMHS SUDS assessment for each client Complete treatment plans and timely treatment plan reviews Ensure all documentation is done as mandated (ROI’s and renewing ROI’s, treatment plans, functional assessments, progress notes within 24 hours of service delivery) Mutually establish session times and locations with clients and families Prepare session content Attend IRMHS Team Meetings Complete Functional Assessments as assigned Case management duties as assigned. This may include, but is not limited to: collaborate with schools, collaborate with probation workers, assistance coordinating care and establishing resources Maintain contact with the referral source as agreed upon Complete session notes, daily time reports, collateral contact notes, family contact notes and incident reports within 24 hours Utilize mileage tracking system to complete mileage and travel documentation within 24 hours. Transition Planning and referrals as needed Complete discharge summary within two weeks of the last session This position is required to participate in rotating on-call shifts, which will include evenings, nights, and weekends

Requirements

  • Current, unrestricted license as an LADC or ADC-T in the State of MN
  • A Bachelor’s degree in the behavioral sciences
  • Travel is a requirement of this position; ability to operate a motor vehicle along with a valid driver’s license is required
