About The Position

The Remote Care Nurse - I plays a critical role in delivering high-quality nursing care to patients through virtual platforms, ensuring accessibility and continuity of care regardless of geographic location. This position involves comprehensive patient assessment, care planning, and coordination with multidisciplinary teams to manage chronic conditions and acute health issues remotely. The nurse will utilize telehealth technologies to monitor patient progress, provide education, and support self-management strategies, thereby improving patient outcomes and satisfaction. By working remotely, the nurse contributes to reducing hospital readmissions and emergency visits through proactive intervention and timely communication. Ultimately, this role enhances healthcare delivery by combining clinical expertise with digital tools to meet the evolving needs of patients in a virtual care environment.

Requirements

• Current and valid Licensed Practical Nurse license in the United States.
• License in a compact state with preference of additional licenses in California and Illinois.
• Minimum of one year of clinical nursing experience, preferably in outpatient, community health, or telehealth settings.
• Basic proficiency with electronic health record (EHR) systems and telehealth technology.
• Strong communication skills to effectively interact with patients and healthcare team members remotely.
• Ability to work independently in a remote environment while managing time and priorities efficiently.

Nice To Haves

• Experience in remote patient monitoring or telehealth nursing.
• Certification in telehealth nursing or related specialty (e.g., Telehealth Nursing Certification).
• Familiarity with chronic disease management programs such as diabetes, hypertension, or COPD.
• Advanced clinical training or certifications such as Certified Case Manager (CCM) or Certified Diabetes Educator (CDE).
• Demonstrated ability to use data analytics tools to track patient outcomes and improve care delivery.
