About The Position
Concordia Visiting Nurses is seeking a relationship-driven professional to join our team as a Liaison. This role is ideal for someone who thrives on connecting with healthcare partners, educating the community, and helping patients access high-quality home health services. This position supports our South Hills and Washington County territories and requires regular travel throughout the South Hills region and Washington County.

- Comfortable networking and developing professional relationships
- Organized, self-directed, and dependable
- Skilled at explaining clinical services in a clear, approachable way
- Adaptable and comfortable working in a fast-paced healthcare environment
- Able to travel consistently within assigned territories
Responsibilities
- Build and maintain strong relationships with referral sources, physicians, and community partners
- Serve as a trusted resource on our home health services
- Educate healthcare professionals on patient eligibility and care options
- Identify opportunities to support patient transitions to home-based care
- Collaborate closely with clinical and operational teams
- Represent Concordia Visiting Nurses with professionalism and integrity
