About The Position

The RN Care Services Supervisor is responsible for managing client care efficiently and cost-effectively to achieve positive patient outcomes, operating within a Licensed Home Care Service Agency (LHCSA). This role is highly focused on Aide retention and includes participation in the Home Health Aide Training Program (HHATP) to teach and mentor new staff. The supervisor will follow Aides from the classroom to the field, ensuring seamless transition and application of learning to hands-on patient care. This role is responsible for developing the overall plan of care for clients and overseeing the outcome of that care, including care delivered by LPNs, HHAs, and PCAs, who must be directed and supervised by the LHCSA RN.

Requirements

  • Current, valid RN license issued by NYS Department of Education required.
  • Minimum of two (2) years’ experience in home care.

Responsibilities

  • Providing and coordinating quality patient care while maintaining an assigned caseload/productivity.
  • Conducting admission assessments to determine safe plans of care, including the need for home health aide/personal care assistance, visit frequency, and duration.
  • Case managing primarily aide-related services according to regulatory guidelines.
  • Conducting in-home supervisory visits to oversee care provided by home health aides and personal care aides.
  • Practicing according to best practice standards and regulatory nursing standards.
  • Maintaining relationships and clear communication with MLTC case managers and other care team members.
  • Communicating effectively with aides, providing mentoring and positive feedback.
  • Overseeing clinical skills of assigned aides and providing training.
  • Communicating with aide preceptors regarding skills and support needs.
  • Documenting according to standards of nursing practice.
  • Contacting primary physicians and/or notifying the clinical supervisor per regulatory guidelines.
  • Mentoring new hires with consistent oversight.
  • Providing leadership, supervision, and operational guidance to Home Health Aides and Personal Care Aides, including reviewing productivity, performance management, employee relations, and disciplinary/counseling.
  • Assisting in completing annual performance evaluations.
  • Investigating and addressing Aide concerns, complaints, accidents, and other issues.
  • Assisting the clinical educator in executing actions for Aide development to ensure proficiency and patient care quality.
  • Ensuring all assigned staff complete annual in-service credits.
  • Assisting in the coordination of the HCR Preceptor program.
  • Demonstrating competencies related to information/technology.
  • Establishing and maintaining relationships with community partners.
  • Performing other duties as assigned.
