Lead Software Engineer - Medical OS

Neko Health•New York, NY

About The Position

Mission Neko is redefining prevention by making data-driven preventative care accessible before symptoms appear. Through a single, non-invasive visit, proprietary technology and direct clinical care provide members with personalised, actionable insights. The Medical OS team builds the software used throughout each Neko visit, from check-in and scanning to the doctor's conversation with the member. The team is developing an AI-first clinical platform that prepares personalised briefings, drafts documentation, and coordinates routine work, while keeping clinicians responsible for clinical decisions. The platform is supported by a longitudinal health record that grows richer with each scan. In the first 6–12 months, the Lead Software Engineer will deliver AI-native clinical workflows across the stack, help develop a clinician-in-the-loop system, and spend time directly with clinicians and members to understand their work and turn those insights into shipped products.

Requirements

  • Full-stack engineering experience and the depth to lead complex, cross-functional delivery end-to-end.
  • Ability to work across both back-end and front-end development, including demonstrating how to close gaps in a less-strong area, potentially with AI.
  • Solid command of architectural concepts and relevant frameworks, with the ability to explain and defend trade-offs.
  • A product-minded, customer-driven track record of shipping well-rated products with excellent customer experience.
  • Experience deciding what to build through direct conversations with customers and participating in product decisions.
  • Demonstrable AI-first working practices, including daily use of agentic tooling and experience building AI or automation into products.
  • Strong communication skills and ability to mentor engineers.

Nice To Haves

  • Consumer (B2C) product experience delivering a polished user experience, or experience with a modern, well-rated B2B or B2B2C product.
  • Med-tech, clinical, or healthcare experience.
  • Experience in regulated industries, such as fintech, and familiarity with regulatory constraints.
  • Exposure to hardware or firmware integrations or device ecosystems.

Responsibilities

  • Lead complex, cross-functional software delivery end-to-end across the stack.
  • Build and ship AI-native clinical workflows, including personalised pre-briefs, drafted debriefs, and intelligent assessment tools, with clinicians retaining decision-making authority.
  • Develop the clinician-in-the-loop flywheel, using clinician expertise to improve the system and helping clinicians spend more time with members.
  • Work directly with clinicians and members, including spending time in clinics, and translate first-hand observations into shipped product.
  • Contribute to product decisions by speaking with customers and determining what to build.
  • Mentor other engineers and communicate effectively across the team.
