Lead RN Case Manager
About The Position
A great healthcare facility located near Rexburg, Idaho, is seeking an experienced Registered Nurse to join their team as the Lead Registered Nurse Case Manager. This is a Full-Time, Permanent, Direct-Hire position with a Monday - Friday schedule in a Home Health setting. The role offers a comprehensive and robust benefits package that includes Medical, Dental, and Physical coverage, as well as a 401(k) retirement plan, Mileage Reimbursement, and much more. The Lead RN Case Manager will develop and update individualized patient care plans, train new team members, model compliance with medication conditions of participation, and act as the primary point of contact for the care team.
Requirements
- Must have or be eligible to obtain an Idaho RN license or Compact RN license upon employment
- Must have or be eligible to obtain BLS Certification
- Must have at least 1 year of acute care and home health nursing experience
Responsibilities
