About The Position

Concerto Renal Services is an industry leader in nursing home dialysis, seeking a Lead Renal Dietitian to join its rapidly expanding team. The role supports the overall policies and goals for the clinical nutrition department, ensuring regulatory compliance, and upholding policies and procedures in accordance with Concerto's standards and state and federal regulations. The Lead Renal Dietitian reports to the Director of Clinical Nutrition.

Requirements

  • Bachelors degree in Nutrition, Dietetics, or similar area required.
  • Registered Dietitian with at least 12 months clinical experience as required by CMS.
  • At least 1-year specialized renal experience.
  • State licensure or certification required if licensure or certification is available in the state where the facility is located.
  • Intermediate computer skills and proficiency with MS Word, Excel, Outlook required as well as functional proficiency with Concerto specific applications within 60 days of employment.

Responsibilities

  • Maintain a caseload of 50–75 patients, providing direct clinical nutrition care.
  • Create and manage monthly unit assignments for RDs.
  • Submit monthly caseload reports for direct reports.
  • Review End-of-Month reports for accuracy and completeness.
  • Perform quarterly audits to ensure documentation accuracy and regulatory compliance.
  • Monitor staff competency, productivity, and clinical documentation standards.
  • Ensure smooth rollout of new processes and initiatives within the region.
  • Troubleshoot regional issues and escalate concerns to management when necessary.
  • Cover units and attend meetings as needed to ensure continuity of care.
  • Attend new unit opening meetings and coordinate appropriate staffing.
  • Coordinate and/or attend new den opening in-services.
  • Obtain out-of-state licensure when needed to ensure coverage.
  • Hire, lead, direct, coach, train, and develop nutrition team members.
  • Supervise RDs within assigned region.
  • Establish recurring one-on-one meetings with direct reports (minimum monthly).
  • Host monthly and/or quarterly RD team meetings.
  • Conduct annual performance reviews and disciplinary meetings when necessary.
  • Ensure team members complete required training (e.g., Concerto University and annual checklists).
  • Review and approve timecards, mileage, and other reimbursements.
  • Submit job requisitions in Paycom for open or new positions.
  • Ensure compliance with federal, state, and accrediting agency regulations.
  • Adhere to facility confidentiality standards, HIPAA regulations, and patient rights policies.
  • Participate in and/or lead departmental Quality Assurance and Performance Improvement (QAPI) programs.
  • Attend monthly QAPI and related meetings.
  • Monitor documentation accuracy and clinical standards.
  • Customize and enforce facility and company policies and procedures.
  • Attend bi-weekly RD Lead meetings.
  • Lead and/or participate in departmental meetings, staff development initiatives, and professional programs.
  • Foster teamwork and implement innovative programs to optimize clinical nutrition outcomes.
  • Ensure patient satisfaction, high-quality care, regulatory compliance, and positive public relations.
