About The Position

The Lead RN Care Management provides leadership to the interdisciplinary team responsible for care management, care coordination and discharge planning. The Lead RN ensures patients move effectively through the continuum of care in an effort to provide appropriate, safe, quality care in a cost-effective manner. The Lead RN acts as a resource for staff and offers input/assists with complex cases, discharge dispositions and other daily issues that occur. Duties include: assistance to the Director of Care Management through oversight and support of staff, case review, scheduling, monitoring length of stay and resource management and Emergency Department Case Management activity when requested. The Lead also provides care management to a defined population of patients, and continued mentorship of the care management team. Ambulatory Lead Care manager will assist Director in oversight of CM caseloads, care plan & assessment completion, potential case identification, and other HealtheCare related tasks. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to team work and cooperation.

Requirements

  • Graduate of an accredited school of Nursing
  • Current licensure as a Registered Nurse in the State of New Jersey or multistate license.
  • BLS required.
  • 2 -- 4 years care management required.
  • Prior clinical experience maybe substituted.
  • Knowledge of AtlantiCare's Value Based Programs/Contracts required.
  • Establishes relationships to influence decisions, gain cooperation, and facilitate change with Providers, all levels of management, employees and supervisors of other Hospital, Ambulatory and community partners required.
  • Well-developed communication and facilitation and analytical skills required.

Nice To Haves

  • Bachelor's degree in Nursing preferred.
  • Certificate for Case Management (CCM) Preferred.
  • Proficiency in Clinical Applications preferred at time of hire; incumbents within position will be trained appropriately and then skill will be required for this position within 30-60 days from date of hire.
