Position: Lead Prevention Services Coordinator Classification: Non-Exempt Reports to: Prevention Services Manager Location: Colorado Health Network Denver Salary Rate: Level 1 (Entry level up to 2 years of direct experience): $26.50 per hour Level II (3+ years of direct experience): $28.47 per hour Bilingual premium for qualifying languages is 3.5% higher than starting salary if pass bilingual exam on first week of hire. Requirements

  • Strong commitment to anti-oppression, social justice, and critical health equity frameworks.
  • Comfortability discussing sexual health and harm reduction strategies through a sex-positive and affirming framework, including topics such as BDSM/kink, condom-less sex, injection drug use, sex work, and gay bathhouses.
  • Ability to uphold respect for client’s autonomy to make decisions about their health and wellness.
  • Flexibility, creativity, collaborative, and a positive workplace attitude in a team dynamic.
  • Strong critical thinking, active listening, and problem-solving skills to address complex situations.
  • Effective verbal and written communication skills as well as high attention to detail.
  • Ability to communicate well with medical providers, community members, and support staff.
  • Basic knowledge of HIV, HCV, STIs, PrEP and other prevention methods.
  • Strong socio-cultural identification or work experience with MSM, LGBTQIA+, and communities of color.
  • Experience in systems navigation, motivational interviewing, and working with confidential information.
  • Bachelor's Degree in Social Work, Human Services, Psychology, Sociology, Public Health, and Communications, a related field or equivalent experience required; candidates with lived experience can substitute educational background.
  • Proficiency with Microsoft Office applications such as Word, Excel, Outlook, and Teams is required.
  • Must be able to work in a diverse setting with diverse populations, including people representative of all gender identities, races, and ethnicities, members of the LGBTQIA+ community, people who are insecurely housed or homeless, people who use illicit drugs, and people who participate in commercial and/or survival sex work.
  • Must be able to work both independently with minimal supervision and as a member of a team.
  • Possesses impeccable written and verbal communication skills.
  • Comfortable with community outreach and coordination with external stakeholders; a positive attitude and an outgoing personality are a must.
  • Organization and time management skills are a must
  • Valid driver's license is preferred for carrying out outreach services.
  • Previous experience in the delivery of evidenced-based programs is highly preferred, as well as excellent communication, client services, and data collection skills.

  • Requires clear speaking ability and the ability to use a telephone for extended periods.
  • Requires ability to view a computer screen (near acuity).
  • Requires fingering agility for keyboarding.
  • The ability to sit, stand, and engage in extended periods of screen time or communication by phone is necessary to perform the essential duties of the role.

Responsibilities

  • Engage in mobile outreach services within the Denver metro area, targeting communities requiring harm reduction services.
  • Coordinate staffing for program requirements, including outreach and educational events, in collaboration with Prevention managers and leads.
  • Develop and facilitate educational presentations in the community regarding NARCAN/Naloxone use, safe injection practices, using drug checking strategies, and other harm reduction best practices. Trainings will include individual and group-level interventions for people who use (inject) drugs (PWUD/PWID), law enforcement, first responders, treatment/health care providers, and other community stakeholders as relevant to overdose prevention practices.
  • Collaborate and communicate effectively with CHN managers and staff to manage workflow, implement programmatic changes and updates, and address any incidents that may arise.
  • Assist the Program Manager in preparing reports to meet grant reporting requirements.
  • Ensure an adequate supply level is maintained to support program needs effectively.
  • Provide in-person support during the operation of the syringe access program, including syringe safe disposal, syringe distribution, distribution of safer injection/snorting/smoking supplies, PWUD/PWID education, data entry, and client registration into the program.
  • Administer HIV/HCV/STI testing and individualized risk reduction counseling services to PWUD, as well as people living with HIV, members of the LGBTQ+ community, people experiencing or at risk for homelessness, and other populations who may benefit from educational programs and services related to HIV, STI, and HCV testing and information, sexual wellness efforts, and harm reduction.
  • Develop and maintain a referral network of PWUD/PWID support and prevention services and provide linkage to care (LTC) as needed.
  • Advance and implement harm reduction education, outreach, supportive services, and syringe access program development in Denver.
  • Represent the organization at community events and collaborative meetings as needed and participate in departmental and agency retreats and meetings.
  • Stay current with best harm reduction practices for people who use/inject drugs and participate in additional trainings when offered.
  • Develop program outcome tracking tools and quality improvement plans to ensure the quality and relevance of programmatic information for PWUD/PWID.
  • Assist in developing and maintaining programmatic tracking and data collection for internal and grant reporting purposes.
  • Participate in the development and marketing of prevention programming that serves PWUD/PWID, people experiencing or at risk for homelessness, incarcerated persons, and other populations who may benefit from educational programs and services related to overdose prevention, HIV, and Hepatitis C testing and information, sexual wellness efforts, harm reduction programs including, but not limited to, syringe exchange programs, and other advocacy and public health efforts.
  • Play a role in social media marketing, development, support, and upkeep, with the goal of informing the community about various programs, incentives, linkage to care (LTC), harm reduction services, referral opportunities, and CHN integrated care services.
  • Collaborate with the prevention services team and Colorado Health Network integrated care services to develop and support programs that are culturally competent and responsive to clients accessing prevention support at Denver CHN.
  • Assist with intern/volunteer/AmeriCorps training and support in the areas of overdose prevention, outreach, syringe exchange, and other prevention services.
  • Develop and facilitate train-the-trainer curriculum to educate peers and other community members on overdose prevention best practices.
  • Offer in-person assistance for CHN's drug checking services, adhering to grant policies and established protocols, and ensuring the maintenance of high-quality information for sharing with the community and all CHN prevention teams.
  • Be flexible with work hours, including working days, evenings, and possibly weekends as needed.
  • Must have current and valid Colorado driver’s license; must be able to pass driving record check per CHN insurance requirements.
  • Must be able to be insured by CHN driving and liability insurance companies.
  • Must have personal insurance on own vehicle if driving on behalf of CHN.
  • Must be able to work in a diverse setting with diverse populations, including persons living with HIV, the LGBTQ+ community, persons of various ethnic backgrounds, disenfranchised communities.
  • Participation in training related to HIV, substance use, harm reduction, trauma informed care and related issues.
